Appointments
A $25 administrative scheduling fee is required to schedule an appointment. This fee will be waived upon arrival for your appointment. If, for any reason, you cannot keep a scheduled appointment, please call as soon as possible to prevent charges from accruing, as this fee will not be refunded if an appointment is missed without notification to our office.

Insurance
Our office accepts a variety of HMOs, PPOs, and other health plans. Please call our office to verify acceptance of your insurance carrier.

* Ankle-Brachial Index - A test to determine the quality of blood flow coming into the feet.
** Epidermal Nerve Fiber Density test - A skin biopsy that looks at if the number of nerves to the feet have been reduced and if the quality of the nerves to the feet has been diminished to determine if a patient has neuropathy and the level of severity.
